Beneficiary for AmeriCorps Seniors SDP Supplemental Accident Insurance

As an AmeriCorps Seniors volunteer, you will be covered by accident and personal liability insurance plus a small death benefit while performing volunteer duties.  This coverage is automatic and free of cost to you as long as you are an active, enrolled member of AmeriCorps Seniors FGP/SCP. Please provide the following information for your insurance beneficiary.

The following information is optional and will not affect your enrollment with AmeriCorps Seniors Senior Demonstration Program.

Occasionally, SDP will purchase volunteer recognition gifts for AmeriCorps Seniors Volunteers.  Please share the size you would use on each item blow.

Volunteers who meet AmeriCorps Seniors Income Eligibility Levels qualify to receive an hourly stipend of $3.25. The stipend is a payment to volunteers allowing them to serve without cost to themselves.

This reimbursement is not considered a wage and is not subject to any tax or charge, meaning it will not affect your benefits.

To receive a stipend, volunteers cannot have an annual combined income, from all sources, deducting allowable medical expenses, that exceeds 200% of the poverty line. Spousal income will also be counted towards the income review. New applicant’s annual income is projected for the following 12 months, based in income at the time of application.

The 2022 AmeriCorps Seniors Income Eligibility Levels are as follows (based on 200% of DHHS poverty guidelines):

For family units with more than four members, add $9,440 for each additional member in all states except Alaska and Hawaii: $11,800 for Alaska, and $10,860 for Hawaii. Income levels are subject to change yearly.

If your combined income sits within these guidelines, you will be subject to an income review before you are enrolled as a stipended volunteer. Once enrolled, you remain eligible to serve and to receive a stipend a long as your income does not exceed the prescribed income eligibility guidelines by 20%. Your income will be reviewed annually.

Stipended volunteers are required to serve a minimum of 5 hours per week.

By signing electronically, I acknowledge that I have read and understand the following statements:

• I hereby state that I am 55 years of age or older and offer my services as a volunteer for the AmeriCorps SDP Program.  I understand that I am not an employee of the AmeriCorps Seniors SDP Project, Meals on Wheels South Texas, or the Federal Government.
• I understand that in my capacity as an AmeriCorps Seniors Volunteer, I may come into contact with confidential information.  I agree to protect this information to the best of my ability and not to disclose it during or after my service as a volunteer has ended.
• I understand that if I use my personal automobile in my volunteer service, I will arrange to keep in effect automobile liability insurance equal or greater to the minimum requirements of the state of Texas. I will also keep in effect a valid Texas Driver's license.
• I understand that assignments are matched to the interests, abilities, preferences, and availability of clients. AmeriCorps Seniors SDP gives special consideration to volunteers and clients with disabilities. If I find that the assignment plan is not matching my interests and abilities, I may be placed with another client or another volunteer service opportunity.
• I understand that I must provide proof of income for the annual income review. If I do not meet the set guidelines, or refuse to provide the necessary information, I may not be chosen to serve again.
• I understand that I must go through the required hours of orientation before receiving assignment.
